Requesting Your Medical Records
OrthoBethesda has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.
Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark. Your medical record request will be processed, and a notification will be sent via email or mail once complete and available for download. There is no charge to the patient to receive their records or images UNLESS the patient would like to rush the delivery of physical records using FedEx. If that delivery method is requested, only the fee for the delivery would be applicable and would be the requesting party’s responsibility. The fee would be $15 for standard delivery and $25 for overnight delivery.
